Thermal runaway is a self-feeding chain reaction in a lithium battery: heat triggers processes that generate more heat, and it keeps going until the battery catches fire or ruptures. Once the reaction has started, stopping it is practically impossible.
The classic trigger is a fully charged pack left in a hot car (60°C and above) or a physically damaged cell. That is why a swollen, hot or dented pack must never be charged or flown.
